QUESTIONNAIRES
Once we receive a pediatric central intake referral, parents, patients and sometimes teachers will be asked to fill out clinical questionnaires. In most cases (parents and patients), questionnaires will be sent directly via secure email to the email address in the patient chart.
For teachers, it is the responsibility of the parent to facilitate the completion of requested questionnaires. Completed teacher questionnaires can be sent to office@nanaimopediatrics.com
To get the most from your appointment, please be sure to complete all requested questionnaires ahead of your appointment time.
